High School and
Middle School athletes are not permitted to participate in off-season,
preseason, or in-season activities/training sessions unless all of
the following forms have been completed and turned into the Athletic Department.
Physicals
Page 1, 3 & 4 must be completed and signed by parent/guardian and athlete. Page 2 must be completed by a physician and is signed and dated. A new physical is required each school year. A physical is good for one year from the date of the exam.

Athletics Handbook & Code of Conduct
The Athletic Code of Conduct must be read reviewed by all Student Athletes and Parent/Guardian. The last page of the Code of Conduct must be signed by the Student/Athlete and the Parent/Guardian. The Code of Conduct should remain in the possession on the Student/Athletes family.

Accident and Sickness Insurance Options
Accidents and Sicknesses happen! When they happen to your child, someone must pay the bills. Here are affordable accident and sickness insurance plans to cover your child either 24 hours a day (24 hour plans) or while in school (at school plan). These plans provide benefits to help meet the cost of medical and hospital expense. If you have other insurance, these plans will help meet the deductibles and coinsurance gaps in those plans. If you have no other insurance, these plans will provide low cost, basic coverage.
